2000 Feet, Inc. Eligibility Requirements:

  • The child must be age 5 to 15 years of age by the beginning of the academic fall semester (August 1st).

  • The parent/guardian must provide proof of eligible child’s age (exceptions may apply for hardships)

  • The child must be experiencing a family hardship or crisis, or

  • The child must currently receive State funded medical care and/or the parent of the child(ren) in the household must receive Food Stamps or TANF (Temporary Assistance for Needy Families).

  • The child must be a first time recipient of benefits from 2000 Feet Inc., in the current calendar year.

  • The parent/guardian must be present with the eligible child(ren) at the designated event.

  • The parent/guardian must provide proof of reason for request either before the event if asking for pre-approval or at the designated event if applying in person.

  • The parent/guardian must agree to the Waiver of Liability, promotional photos/videos (when applicable)  and the statement of truth by reading and signing the Agreement Acceptance on the application.

Important!

  • Any information on the application is subject to verification. Send all verification with your application.

  • You will be notified of our decision of your request via email first. If you do not have an email address please provide an email address of a family member, friend, social worker, etc. who will notify you of our decision.

  • Due to the volume of requests we cannot always guarantee a phone response.

  • If you have not received a response from us 24 hours prior to the event, then the event is closed and has reached its maximum pre-approvals or you were not approved for the benefit.

  • You may be denied benefits from 2000 Feet, Inc., for knowingly providing false information and/or registering more than twice per calendar year. Review the full details of each event before submitting an application:

Proof of Reason for Request:

You are not officially approved to receive benefits from 2000 Feet, Inc. until the following documents have been submitted as proof of need. If applying online the documents must be sent as either an attachment via email, sent direct mail or faxed to 636.447.6362. Please provide the parent’s full name with the documents if submitting separately. If applying in person the original documents must be disclosed at the event.



Proof of a hardship or crisis: (hardships or family crisis are given priority)

Hardships may be the result of a unique family situation. Problems that are common to large numbers of families do not constitute a unique family hardship. Similarly, medical problems such as asthma and motion sickness that are experienced by any children do not constitute a unique hardship. Family hardship appeals will only be considered for families who have a unique serious, documented hardship. Circumstances can only be considered for a child living in the same household.



Examples of a temporary hardship or family crisis

a.       Loss of job of one or both parents in the household

b.      Natural disasters, e.g., flood, tornado, hurricane, fire

c.       Homelessness

d.      Foreclosure on the home of the family’s principle place of residence

Child(ren) must be school-age 15 years or younger (exceptions may apply and proof of age is required).

The parent/guardian must provide 2000 Feet, Inc. with an original letter stating the family’s hardship or crisis. The reason for request must be written on the institution’s letterhead or stationery and must come from either/or:
a)     The school’s guidance counselor,  social worker or nurse where the child is assigned

b)     The family’s case manager

c)      The family’s Pastor

d)     A doctor’s statement


Proof of Child’s Age/Date of Birth (DOB):

  • A certified birth certificate,  or

  • A certificate of US Citizenship, or

  • Native American Identification card, or

  • Any State Funded health care card that reflects the child’s name and Date of Birth


Proof of Government Assistance:

  • Currently receiving MO Health Net for Kids (current card for each child)

  • Currently receiving Missouri  Medicaid (red medical card)

  • Illinois Health Care (current card for the current month of the benefit)

  • Currently receiving Food Stamps (must provide the letter of approval from the Missouri Department of Social Services stating the benefits)

  • Currently receiving TANF- (must provide the letter of approval from the Missouri Department of Social Services stating the benefits)

To Apply in Person:


There are a limited number of available benefits that may be given away at the event. Priority will be given on an eligibility basis as First Come, First Served.

Please note: To guarantee that you will receive the benefit, you must be in the limited number of pre-approved applicants via our online application.

You are entitled to fair and equal treatment regardless of your age, sex, race, handicap, religion, creed, national origin or political belief.

All benefits are based on availability of funding.

We respect your privacy. Be advised that materials that you have submitted to 2000 Feet, Inc., in evidence of your family’s unique family hardship or request is kept confidential and is not forwarded or shared with any third party. Unapproved requests are not kept on file and are shredded.
